Cost estimates
Due to the variety of services we provide and the wide range of medical needs our patients have, it is difficult to project exact cost estimates for your care. The registration staff in your clinic can give you a general fee estimate and answer any other fee or payment questions for you.
Financial Assistance
There are programs available for patients who do not have insurance or require additional financial assistance. Contact the Central Financial Screening Office, 743-1970, ext. 268, or 800-872-2956 for more information.
Payments
Co-payments, payment of non-covered services and payment for services for self-pay patients are expected at the time of service.
Payment Options
We accept most major commercial insurance coverage, as well as Medicare, Medicaid, cash, check, and credit cards (American Express, MasterCard, Visa, & Discover). Self-pay payment and insurance copays are expected at the time of service. A patient is considered self-pay if he/she does not have any form of insurance. Copay is the amount an insurance provider requires the patient to pay. This amount will vary depending on services provided and insurance providers. Due to the wide variety of services we provide, it is difficult to project exact cost estimates. Clinic personnel can provide a general fee estimate but cannot quote an exact cost.
Patient Confidentiality
